    To whom it may concern,

    I am Erica Mitch V. Bernalte of BS Biology 1A. Please accept this letter as my formal
    excuse for not attending the meet for the performance of the Strength and Mobility Training
    activity last Tuesday, June 7, 2022. I had been experiencing recurring menstruation cramps and
    the pain became unbearable due to my hormonal imbalance and I need to take medicine to lower
    the pain. Relative to this, I humbly request your permission to excuse me from my class.
    I am deeply sorry. I hope you understand my situation and the cause of my absence.

    I have attached here below, the copy of my medical prescription last year for proof and
    future reference.
