To whom it may concern,

This letter is to address the inter-office memo posted on June 20, 2023 requiring the reason to
why I could not attend the scheduled training. 

I, Karylle Louise M. Samson, a registered Medical Technologist at the Pathology Department at


Diosdado P. Macapagal Memorial Hospital, would like to sincerely apologize for not being able to attend
the scheduled refresher training for the Hematology Automated Analyzer (XN 1000) on July 19, 2023 for
the reason of having a recurrent diarrhea suspected due to my history of amoebiasis. 

On July 18, 2023 after I got home from my 12-hour duty, my family decided to dine out for the
celebration of father's day. We got home pretty late. Around midnight, I had a stomach ache and
eventually led to diarrhea. I took some pain medicine and imodium but unfortunately it did not work. I
also took dicycloverine, which is my medicine for my amoebiasis back then. I may have consumed
something during the celebration of father's day that triggered my condition. I would also apologize for
not giving a notice sooner; I did not get enough sleep that night and woke up late that day. 

I decided to prioritize my health and have a proper rest to restore my well-being especially
because I have a day duty again for the next day (June 20, 2023). The day of the training is my only off
and I don't want to risk it because I might not be able to go to my duty if I am still sick. I don't want to
leave my other colleagues because they will have a hard time due to being short-staffed in a long and
exhausting 12-hour duty.

I did not intend to disrespect or offend anyone for my absence, moreover, to cause
insubordination. Once again, I sincerely apologize for my action.
