DEAR SIR,

I would like to inform you that I am resigning from my position as VETERINARY NURSE at ANIMAL HOUSE
VETERINARY HOSPITAL –Paranaque branch effective February 16, 2023. For the reason that I have a
health issue and based on my CBC result my hemoglobin dropped, which is why I always feel dizzy and
often have a headache. So my parent and I agreed to take a rest at home first.

Thank you for the support and the opportunities that you have provided me during the last 6 months. I
have truly enjoyed my tenure with Tags Manpower Services and Animal House Veterinary Hospital, and I
am more than grateful for the encouragement you have given me in pursuing my professional growth
objectives.
