    Dear Ma’am/Sir,
    I, Clint John D. Fernandez from Grade 9- Honesty , Please accept my sincere apology for
    being unable to attend my class for today March 17 because I have to go to the hospital for my
    laboratory examination. I will also be absent this March 20 because it’s the schedule for my
    surgery, it will also take days for me to go back to school because I need time for it to heal.
      
    The hospital medical certificate will be released tomorrow after my laboratories result. I am
    enclosing my medical record after its release.
