My name is Clarizza Joy J. Cuizon, and I am a shifter student from the College of Education, now in my
second year of pursuing a Bachelor of Science in Psychology. I am writing to request that my
application for the Biochemistry course be reconsidered. As data suggests that the said course has
been not applied, which I am ignorant of, I request that you grant my request to include the
Biochemistry course that I attended and completed this semester in the data portal.

I agree that I made a mistake by not visiting my student portal on a regular basis, but I can tell you that
it will not happen again. I am a hardworking student who takes my academics very seriously, therefore I
humbly request your forgiveness and hope you decide to give me a second chance, as well as your
kind consideration.
