LEGAL AID ONTARIO WILL NOT CONSIDER APPEALS THAT ARE RECEIVED
AFTER 15 days from the date of this notice.

3. Lawyer's opinion letter or relevant correspondence;
4. Any transcripts; and
5. Any further material you may submit in support of your appeal.

financial reasons, and your income has gone down since your last assessment, you can contact the Client

I Robert Bell wish to appeal/request a review of the decision of the District Area Director at Client Service Centre
Office made on 17 March, 2021 .

The reasons for my appeal/request for review are:

*If you would like a Request for Review based on debts and/or medical expenses, please submit
the following:

proof of current debt(s) and proof of current payment of debt(s). Debts are ongoing monthly payments to
items such as credit cards and loans. Payments for services such as car insurance and utility bills are not
considered debts.
proof from your pharmacist/health care provider confirming current cost for medical expenses for the past 3
months. Medical expenses are ongoing expenses for treatment that is not covered by your insurance plan or
any other provincial healthcare plans.
