ward of Rajavithi Hospital. Assigned to take care of Mr. Mark, 62 years old, his severe chest pain
to scapula has increased. Was transferred from Samut Prakan Hospital to the Coronary care unit
ward at Rajavithi Hospital in the meantime.

I am referring the patient to you for treatment. He is The patient’s name is Somchai
jaidee ,male. Date of Birth: 16 April 1962 Age : 62 years ,
Nationality: Thai ,Race: Thai , Buddhism, Height: 178 centimeters, 65 kilograms, Marital Status
Married, Occupation: Motorcycle taxi rider

He had both symptoms of tightness in his chest. The side was chest pain radiating to the
scapula. along with sweating a lot and cool skin while watching TV So he came to the hospital
and gave a history of smoking 10 cigarettes/day, drinking 1 small bottle of alcohol/day, denying
any congenital diseases, drug allergies, food allergies.
First symptoms: Feeling good, asking and answering questions. There is severe chest pain
radiating to the scapula in the back, pain score 8/10 points.
V/S : BT 36 oc, P 78 bpm, RR 20 bpm, BP 116/63 mmHg , DTX 127% , O2sat 92% gave oxygen
cannula 3 LPM, did EKG 12 lead, found ST elevation V2-V5, doctor gave fluid 0.9% NSS 1000ml
vein drip 80 ml/hr. Received blood clot dissolving medicine. (Streptokinase) 1.5mU in 1 hr.
