DOCTOR’S ORDER SHEET

The physician order sheet is a direction directed at medical personnel available to follow up on
the procedure given by the person authorized to write on the physician order sheet. The contents
of this sheet must be carried out and followed in accordance with the written details. The text
contained in the physician order sheet must be able to be understood and read by all medical
personnel who are responsible for implementing it so that there is no miscommunication.

The physician's order contains several details that must be in the sheet. Without these details,
orders cannot be processed. The first is a referral. The reference contains the reasons and
description of the disease and tests that have been done before. After that, there will be a name
from the patient and part of the patient's identity needed such as date of birth and so on. Finally,
for a physician order sheet to be truly valid, a signature is needed as proof that someone is
responsible for the order that was given. If one of the details is not fulfilled, the sheet must be
given back to the person who gave the order to be completed so as not to create an error in the
action given by the medical personnel.
