etc. and in case of tenant, NOC from Landlord/Lady. However, in case landlord does
not issue NOC, then in case, witness from 2/3 neighbors be attached to the effect
that he & her members of his/her family are residing at that address & for how
long. 14. Whether self “Affidavit” submitted by applicant (HOF) is in order. (i)
Whether non-Judicial Stamp Paper has been purchased in his own name. (ii) Whether
the date of purchase of Non-judicial Stamp Paper is not later the date of execution
of the Affidavit. (iii) “Verification” has duly been done by the executants on the
Affidavit. (iv) Affidavit is duly attested either from SDM/Notary Public/Oath
Commissioner (if by Notary Public, then govt. Revenue Stamp of Rs.5/- is affixed on
it). 15. Whether old ration card has been enclosed with the application, if not,
reason be mentioned. In case of loss of ration card, whether copy of FID u/s 155
Cr.P.C. in the prescribed format enclosed and whether the FIR is in the prescribed
proforma. 16. Whether Personal ID has been enclosed i.e. a self attested photocopy
of latest paid bill of Telephone, Water, Electricity, house Tax, Driving Licenses,
Passport. Election ID card, Bank Pass Book, Employment ID Card. 17. Whether
undertaking has been given that he or any of the members of the family have no
other ration card in Delhi/outside Delhi and their names are also not included in

18. Whether the proper physical verification report has been submitted alongwith
the total number of Units (adult and Minor) by the Area Inspector, 19. Whether the
Circle FSO has recommended the physical verification report submitted by the Area
Inspector. 20. Whether name of members of the family proposed to be included in the
ration card are Blood Relation or not ( in case of doubt, some documentary7 proof
may be insisted upon). However, maidservants kept in the house/khoti, though, do
not fall in the category of “blood relation”, they have been allowed to be included
as members in the ration card as per instructions issued by the Department. 21. All
