SHE PROGRAM

23.0 HEAVY EQUIPMENT HANDLING

23.1 PURPOSE
The purpose of this document describes the minimum requirements for safe operation, inspection and
maintenance of mechanical and heavy equipment.

23.2 DEFINITIONS
Barricade – is a barrier erected or laid around the work area to prevent or delay the movement of
opposing forces.
Work permit – a formal document that authorizes a group of workers to perform their certain task at
a particular place and time.

23.3 RESPONSIBILITIES
Supervisors

The Supervisors are responsible for ensuring that practices identified in the General Safe Work
Practices (GSWP) are applied by the personnel under their supervision.

Rigger

A rigger (levels I, II, and III) are responsible for safe controlling of the load that is being lifted.

Flagman

The flagman is responsible is to guide the heavy equipment and controlling the traffic in order to
avoid motor-vehicle accidents (MVA) that can cause harm to both plant and the personnel
around.

23.4 GENERAL SAFE PRACTICES


 Mechanical and heavy equipment shall be inspected on a regular basis by a competent heavy
equipment inspector or mechanic
 The inspection shall be conducted on all mechanic and heavy equipment prior to their use. Users
performing inspections shall ensure the following, at a minimum, prior to operating the
equipment.
o Fuel, oil, hydraulic fluid, water, etc., leaks/spills shall be immediately contained, cleaned
up and properly disposed.
o Hose and pipe connections checked for wear and cracks. Defective hoses/pipes shall be

replaced or repaired.
o All lights and warning devices operate properly.

o Wiring inspected for damage or improper repairs.


o Fire extinguishers inspected, tagged, operable and accessible.
 Only the authorized personnel shall be allowed to enter the work area where the mechanical
and heavy equipment is in operation. When entering the work area, authorized personnel shall
make direct eye contact with the equipment operators to ensure their presence is known.
 Operators shall ensure the work area is clear prior to the operation of mechanical and heavy
equipment.
 Reverse alarms shall be audible and working on all heavy equipment.
 The flagman must be present and visible by donning a high-visibility fluorescent vests in order
the guide the heavy mechanical equipment.
 Personnel shall not rest or sleep (e.g. seek shade) under the mechanical or heavy equipment at
any time.
Crane and Boom truck

 The crane and boom truck must be inspected, approved and third party certified.
 The valid work permit must be issued and present at the work area at all times.
 Specific crane must be used during the lifting activity as specified in the work permit.
 All crane components and the rigging tackles must be inspected and in good working condition
prior to the execution of the job.
 All cranes shall be provided and fitted with load/radius indicator chart.
 All lifting tackles such as chain blocks, pulley wheels, and other similar devices shall have the
maximum safe working load clearly marked upon them.
 The slings and other lifting gears must be in good condition.
 The lifting operation shall always be under the direct guidance and signals from an authorized
and certified rigger.
 The crane operator must possess a valid SAG license and third party operator certificate.
 Any personnel is prohibited to stand or walk under the suspended loads. Barricade the lifting
area with signboards posted to avoid any unauthorized entry.
 Outriggers must be fully extended at all times during the lifting operation.
 Taglines shall be used to control the load.

Forklifts

 Forklift must be regularly maintained and inspected prior to use.


 Forklifts shall be used in accordance with the manufacturer’s instructions.
 Forklifts with additional special equipment shall meet the manufacturer’s specifications.
 Forklifts shall have a valid inspection sticker.
 Overhead protection, seatbelts, fire extinguisher, visible lights, horn and a backing
warning alarm shall be equipped.
 Forklifts shall only be used on stable road/surface conditions.
 The forklift operator shall possess a valid driver’s license as per the local rules and
regulations for the forklift operation.
 Stunt driving and horseplay shall not be permitted.
