CHIEF DIAGNOSIS : UPPER RESPIRATORY TRACT INFECTION OF SORTS

This is to certify that Mr. FUNGO, JOHN MARK [22M], has been diagnosed with an Upper Respiratory Tract Infection (URTI) and
since March 22, 2024.

Clinical Assessment and Clinic Findings:

1. Diagnosis:
- Upper Respiratory Tract Infection (URTI)

2. Assessment:
- Patient presents with symptoms such as cough, nasal congestion, sore throat, and fever, indicative of an Upper Respiratory Tract
Infection.
- Additionally, the patient experiences diarrhea characterized by loose or watery stools, abdominal cramps, and dehydration.

**Treatment Plan:**
- For the Upper Respiratory Tract Infection (URTI):
- The patient is advised to rest and stay hydrated.
- Over-the-counter medications such as acetaminophen or ibuprofen may be used to alleviate fever and discomfort.
- If symptoms persist or worsen, further medical evaluation may be necessary.

**Remarks:**
- Mr. FUNGO, JOHN MARK. is advised to stay home and rest until symptoms improve. Bedrest 1-2 days.
