I hereby certify that I have examined:

Mr. Leonardo M. Gonzales, 19 years of age, of Stamford Residences, Mckinley Hills,

Taguig City

And find that:

After a thorough check up, and examination, the patient, whose name appears above, has
been found to have been suffering from acute duodenal petic ulcer, and as such he has been
experiencing fever extreme burning stomach pains as a result of the infection. Furthermore,
the patient also has been feeling bloated, and undergoing intolerance to fatty food. After
diagnosis, the patient is therefore required to undergo treatment and medication, to have bed
rest for the next 3 days at least, or until such time he has been cleared to go back to work by
this doctor. Its is absolutely necessary that the patient follows strict medication as provided by
this doctor, to avert his condition from progressing into ulcerative colitis.

This certificate is provided after the patients check up on October 28, 2017 for whatever
purpose it may serve.
