INSTRUCTIONS ON HOW TO CLAIM YOUR TEST PERMIT

Dear MARK GIAN CARIÑO,

Your test permit for taking the UPCAT 2024 will be available for release on May 04, 2023
from 1:00 PM - 5:00 PM at the UP Office of Admissions.

To claim your test permit, you need to bring the following:

(1) a printed copy of this email
(2) your school ID

We advise that you also bring your personal supply of water to keep you hydrated and an
umbrella for weather protection.

In the event that you are unable to claim your test permit in person on the assigned
schedule, you may authorize someone to do it for you. Your representative must bring the
following to claim the test permit on your behalf:
(1) an authorization letter signed by the applicant
(2) a photocopy of the front and back of your school ID
(3) a printed copy of this email
(4) a photocopy of the front and back of the representative's ID

PLEASE CLAIM YOUR TEST PERMIT ON THE ASSIGNED RELEASE DATE.

THE OFFICE OF ADMISSIONS WILL NOT ENTERTAIN THOSE WHO ARE NOT
SCHEDULED ON THE DESIGNATED DATE.
We are also giving you the details of your UPCAT 2024 exam schedule as follows:
