Greetings!
I am (Parents/ Guardian Name) writing this letter to provide consent for my child,
(Student’s Name), a Grade 12 Humanities and Social Science student at University of the
Philippines to volunteer with your organization. I hereby give my permission for my child to
participate in the activities organized by the Greenshare.Ph Organization, under the
supervision of responsible adults. I am aware that my child will be involved in several tasks
assigned by the organization, and I trust that adequate measures will be taken to ensure their
safety and well-being during their time as a volunteer.
I understand that my child will represent our family, hence I will ensure that my child
will abide by the rules and guidelines of the Greenshare.Ph Organization. I guarantee that my
child is punctual on volunteer duties and provide the necessary support to help them fulfill
their responsibilities.
Furthermore, I authorize Greenshare.Ph Organization permission to administer my
child's medical treatment in the case of emergency while they are volunteering. However, I
am aware that the Greenshare.Ph Organization does not have a budget for hospital fees,
therefore, I will be responsible for the expenses for the necessary medical care. I will provide
the organization with any relevant medical information or special instructions that they may
need to ensure my child's safety.

you require any further information or if there further forms that my child needs to fill out to
complete before she or he can begin volunteering at your organization. Additionally, these
