Alleged Defendant (hereinafter, “Accused”) requests discovery and inspection with the right to copy or
photograph, where appropriate, the following material:
1. Any written or recorded statement made by the Accused discoverable
under the doctrine of discovery pursuant to Public Law.
2. Any summary of an oral statement of the Accused discoverable under the
doctrine of discovery pursuant to Public Law.
3. The recorded testimony, transcript, or police body cam videos of any testimony of
the Accused discoverable under the doctrine of discovery pursuant to Public Law.
4. A copy of the prior criminal record of the Accused discoverable under the
doctrine of discovery pursuant to Public Law.
5. All documents, tangible objects and physical evidence discoverable under the
doctrine of discovery pursuant to Public Law.
6. The results or reports of any physical or mental examination or scientific test or
experiment discoverable under the doctrine of discovery pursuant to Public
Law.
7. A list of the names and addresses of the witnesses whom the Prosecuting Attorney
intends to call at the trial of this cause, together with any prior felony record of
such witnesses, discoverable under the doctrine of discovery pursuant to Public
Law.
8. All evidence favorable to the Accused and material to the guilt or punishment of
the Accused, discoverable under the doctrine of discovery pursuant to Public
Law.
9. A copy of the arresting officer’s notes, including definitions for all abbreviations
of words and/or terms written in the officer’s notes.
10. A copy of any reports or investigations performed by the arresting officer, or
other officers on scene at time of arrest, that were made in connection the
Accused being engaged in commerce with his private automobile at the time of
the incident.
