I, GESTINE L. SARMIENTO, of legal age, married, Filipino citizen, and with
postal address located at 405 Maribel Subd., Canlalay, City of Biñan, Laguna, after
having been duly sworn to in accordance with law, hereby deposes and states;

That I am the owner of a business under the name of GS MILKTEA SHOP under
business Permit No. 2023-07772 located at Block 5, Lot 42, South City Homes, City of
Binan, Laguna.

That for the year April 2023 to December 2023 my Gross Income is FIFTY
THOUSAND PESOS, (PHP. 50,000.00) Philippine Currency;

That this affidavit is being executed to attest to the truth of the foregoing facts for
whatever legal purposes it may serve me best.
