Dear Dr Stevens,
Re: Ms Sally Fletcher, 25-year-old

I am writing regarding Ms Fletcher, who is ready to be discharged today for further follow-up,
after undergoing laparoscopic surgery for an endometriotic cyst.

On 25/03/2018, Ms Fletcher admitted herself with complaints of painful periods for 3 years.
She was diagnosed as a case of endometriois. Her transvaginal ultrasound showed 6cm cyst
for which she underwent this operation laparoscopic surgery with uneventful recovery.

Regarding Ms Fletcher's post-operative care, her incisions need to be kept clean and dry. In
regards to. Regarding her nursing care, Ms Fletcher should be she was encouraged to take
oral fluids, resume her regular diet as early as possible and Moreover she also need to be
ambulate as per her tolerance.

She. Ms Fletcher is stable and is well-oriented to time, place and person. Her post-operative
blood investigations are unremarkable with good overall progress. She is allowed to take
shower after 24 hours of discharge. In addition, she can resume sexual activity after 02 weeks
of surgery and driving can be resumed after 24-48 hours after completion of the final dose of
analgesics.

In view of the above, it would be appreciated if you could follow up on Mrs Fletcher’s
condition at your clinic.

 Write the second name of the patient with Mr/Ms instead of the pronoun at the
beginning of each paragraph. If the patient is less than 16 years old, you only write the
first name without Mr/Ms. If pt is over 16 years old, write 2nd name with Mr/Ms, Ms
Fletcher.

 Improve your case note selection by focusing on presenting complaints of the patient’s
most recent visit and by keeping the recipient of the letter and his profession in mind (to
whom you are writing the letter).

Some connectors are less suitable for healthcare letter writing. These include
