ANNOTATION

I conducted a home visitation last December 24, 2021 to inform his guardian that he is not
answering some of his modules. When I noticed it, I found out that he is poor in reading
comprehension. Upon our conversation with his guardian we agreed that if he had time he can go
to school and I taught him how to read. I also gave him reading materials that he can read at
home. His guardian told me that they will also taught and guide him how to read and answer his
modules. As of now he was able to answer and pass modules on time and I noticed that he
improved in reading comprehension.
 I conducted a home visitation last February 10, 2021 to Jomer D. Marcelino to inform
his parents that he is not getting/ unable to finish the given activities in the modules. Upon our
conversation with his parents that I gave him enough time to finish and next time he must
answer completely his modules. They said that they also guide their child in answering the
modules.
