PETITION FOR NOTARIAL COMMISSION

COMES NOW, the undersigned petitioner, unto this Honorable Court,

most respectfully avers:

1. That petitioner, is of legal age, Filipino, Single, a resident of San

Bartolome St, Sipocot, Camarines Sur and maintains a regular place of
work at Gabon Reyes Law Office, South Centro San Bartolome St,
Sipocot, Camarines Sur;

2. That he is over twenty-one years of age having been born on the 12th
day of November 1976 in Sipocot, Camarines Sur;

3. That he satisfactorily completed the required course of law at the

University of Nueva Caceres in June 24, 2024;

4. That he passed the 2024 Bar Examination and was admitted to the
Philippine Bar on December 21, 2024, under Roll Number 85895 per
certificate of Membership by the Philippine Bar Confidant, Supreme
Court of the Philippines, a copy of which is hereto attached as Annex
‘A’.

5. That he permanently resides at San Bartolome, Sipocot, Camarines

Sur, a place situated in the province where he desires to be
commissioned as a Notary Public;

6. That he desires to be commissioned as Notary Public for and in the

Province of Camarines Sur under the 2004 Rules on Notarial Practice,
and that he possess all the qualifications and none of the
disqualifications for said office.

 7. That he has duly paid the requisite fees for his profession as shown by
IBP Official Receipt Number 255441 on the 7th day of January, 2025 and

 Professional Tax Fee Official Receipt Number 5906789 on the 4th day of
January, 2025 and as certified by the Integrated Bar of the Philippines,
Camarines Sur Chapter, copies thereof being herewith attached as
Annexes ‘B’ and ‘C’ respectively;

8. That he is a member of the Philippine Bar in good standing and that

there is no disbarment case file against him as cleared and certified by
the Office of the Integrated Bar of the Philippines, as evidenced by
Annexes ‘D’ and ‘E’;

9. That he is a member good standing of the Philippine Bar and that no

pending administrative case is file against him before the Office of the
Bar Confidant of the Honorable Supreme Court. The certification
pertaining such matter is hereto attached as Annex ‘F’.

10. That petitioner has paid his membership dues as evidenced by a

Certificate of Payment of Membership Dues issued by National
Treasurer of the Integrated Bar of the Philippines, original copy of
which are attached hereto as Annex “G”;

11. That he has never been charged or convicted of any crime involving
moral turpitude. There is no pending administrative or criminal case
filed against him as certified by the Clearance issued by the Regional
Trial Court dated Octuber 28, 2024, attached herewith as Annex ‘H’;

12. That he hereby attaches as required his three (3) passport size
pictures colored photographs taken within thirty (30) days from the date
of the filing of the instant Petition, as Annex ‘I’;

13. That he submits his specimen signatures, attached herewith and

made an integral part hereof as Annex “J”.

14. That he has duly paid the filling fees for the instant petition as
required by the Rules, with Official Receipt thereof duly attached as
Annex “K”.

15. That he submits a location map where he will hold his notarial
services, attached hereto as Annex “L”.

P R A Y E R

WHEREFORE, premises considered, it is most respectfully prayed of

this Honorable Court that the undersigned petitioner be granted a
commission as Notary Public for and in the Province of Camarines Sur.

accordance with law, do hereby depose and say;

of this affidavit;

2. That I am a practicing lawyer by profession with office address at South

Centro, Sipocot, Camarines Sur;

3. That as of date, I am applying for my notarial commission for and in the

Province of Camarines Sur and is filing a petition thereof with the Regional
Trial Court of Camarines Sur;

4. That, I intend to perform notarial practice for and in the Province of

Camarines Sur and hold office at my permanent residence at Sipocot,
Camarines Sur.
