Honorable Michael Lee Chin,

Reference is made to your letter Re: Facilities held with National Commercial Bank Jamaica Limited dated
December 20, 2023 sent via email and Registered Mail signed by your agent Tarik Gray.

Please be made aware that you have failed to provide the requested and necessary PROOFS OF CLAIM
after receiving my letter of conditional acceptance dated November 15, 2023.

Your response was inadequate and has failed to provide:

1. Validation of the debt—that is, the actual accounting.

2. Verification of your claims against me; that is a Signed Invoice.

3. A copy of the Contract binding both National Commercial Bank Jamaica Limited and I.
As the Respondent, you are now in fault. You have the right to cure this fault by providing the proper
documentation within ten (10) days from the receipt of this Notice of Fault.

Please send the sufficient documentation using Jamaica Post, Registered Mail and signed with a wet ink
signature under full commercial liability and penalties of perjury, assuring and promising me that all of
the replies and details given to the above request are true and without deception, fraud and mischief.

Your said failure to provide the aforementioned documentation within ten (10) days from the above date
will constitute acquiescence.
