Phase 2 Lesson 1

1) Make sure that you have a sub-folder called Phase 2 in the PAT folder.

2) Copy your report that you created in phase 1 to your phase 2 folder.

2) To collect data and information that you may not find in other sources
or to support data or information from other sources, you need to ask
people questions. To do this you will need to compile a questionnaire.

Under the Phase 2 folder, create a Word document called Questionnaire.

To create a new Word


document:

Right-click in the phase 2


folder and then

3) In the above Word document, you will create a questionnaire.

Make use of Legacy tools for the "answer boxes".

How do I get legacy tools?


 File, options
 Quick access toolbar
 All commands
 Search for “legacy tools” and “lock” and
add.
Remember that questions with only one answer or questions where people are able to
choose an answer from a list are often easier to process.
Questionnaires must have headings and clear instructions for users.
Group related questions together under appropriate headings.
Make use of professional formatting.
Create a questionnaire with at least FIVE questions, excluding biographical
data (e.g., name, gender, age, address) of the respondent.
It is important that you include questions that will give you data (numbers, quantities,
statistics, etc.) that you will be able to process in a spreadsheet afterwards.
For example: On a scale of 1-10…..
Some questions must have numbers as an answer, because you will need it to draw a
chart later on in Excel.
The questionnaire must fit on one page.
