Introductory Message

For the facilitator:


This stand-alone module in TLE (INFORMATION AND COMMUNICATION
TECHNOLOGY is intended for distance learning modality. Learners who could not afford
online or face-to-face learning can utilize this module. It includes a learner-friendly pretest,
self-check exercises, additional activities, and other learning instructions intended to help
learners learn even teachers and learner are geographically remote from each other. This
covers topics in Grade 6 in the 1st Quarter with corresponding learning activities for
learners to achieve the most essential learning competencies and lesson objectives.
Moreover, each exercise in this module is designed for independent learning and practice.
However, ensure that the learners’ parents, elder siblings, guardians or relatives at home
will guide and support them as they accomplish its given tasks.
Furthermore, explain to students that taking the tests diligently will allow them to
learn their lessons for their academic progress. Most importantly, remind them to answer
the given activities on a separate answer sheet and handle this module with utmost care.

This learner-friendly module in TLE is specially designed for your learning needs
and progress. Your patience and dedication in reading and answering its learning
exercises will help you successfully cope with lessons. You are free to ask assistance
from your teachers, parents, siblings, friends, and family members whom you think can
help you best. Read each lesson carefully and follow the instructions for your activities
after reading.
Remember to answer the given exercises in a separate sheet of paper. When you

What is an online survey?

Online Survey is a study that aims to gather data to


get the particular view, belief, or opinion of a group of
respondents about a specific issue or topic. This survey is
conducted through the internet using a particular website.
This is made and administered survey since you are
the one who formulated the survey question/item, hence
mad your own survey form posted it on the website of your
choice collected or gather answer (or data) from your
respondents, and upon gathering data, tabulated them.
By gathering and tabulating the data or answer of your respondents on the survey
questions you can come up with your findings and arrive at conclusion about them.

Online survey tools


1. SoGoSurvey for one simple reason—as far as free
survey tools go, you won’t find another that delivers so
much value.
GoSurvey is an end-to-end survey design, distribution,
and analysis platform. Because it provides a full suite of

2. Survey Monkey it is considered the most well-known


online survey tool. It has a free version and a paid version.
The system is well designed, pretty easy to use, and you can
embed surveys
In the free version, you’ll get:

b. 15 question types
c. 100 respondents
d. Light theme customization and templates

3. Google Forms these are free online survey tools that can be
sent to unlimited respondents. Data automatically go to
Google Spreadsheets so they can be analyzed easily. Using
Google forms a Gmail Account.

4. Typeform wins the championship belt for creative


survey creation. Type form’s interface is elegant and its style is
attractive, reflecting the company’s motto, “Asking questions
should be easy, human, and beautiful.”

How to Create a Survey Using Google Forms

Google Forms gives users an easy way


to create a survey that contains as many
questions as they need to ask, in a
variety style. From planning an event
to getting anonymous answers to tough
questions, there are a ton of useful things
you can do with Google Forms.

From multiple choice questions to a linear


scale, Google Forms gives you a handful
of options for asking questions. You can
also decorate your survey to fit its theme
and opt to make certain questions
mandatory.

While Google Forms surveys are typically sent and answered via email, you can
also have people fill in answers on a web page, embed the questionnaire on a site and
share it via social media. Here are our step-by-step instructions for how to create a survey
with Google Forms.

Creating an Online Survey Form


The following are the steps in creating and disseminating an online survey form
using Google form.

1. Navigate to https://docs.google.com/forms/
and click Blank. Google Forms has a number
of pre-made templates to choose from, and
you can view them all by clicking more.

2. Name your survey. You can also add a


description. If you want to name the Google
Form for your own reference, click untitled
form in the top left corner to edit.

3. Tap on Untitled Question and write a


question.

4. Click multiple choice

5. Select an option for how the question will be


answered. For all options except for Short
answer, Paragraph, Date and Time, you'll
have to write in options for answers.

6. Click the side menu icons to add to your
survey.

7. Click the Required switch to make a


question mandatory.
8. Repeat steps 3 through 7 as necessary.
9. Click the duplicate or trash icons to clone or
erase the

11. Click the Gear icon to access survey
setting
12. Click Send.

13. Enter recipients. Check off "Include form in


email" if you want your respondents to
answer questions from their email client.
Not all clients support this. Outlook, for
example, will make you click a button to
open the survey in a browser.

14. Fill in a subject line and message. People


typically need a little coaxing to answer a
survey.

15. Click Send. If you want to share the survey


via a hyperlink, it can be found by clicking the
link icon. To get code for embedding the
survey on a website click the <> icon. You can
also share the survey via social media with
the Google+, Facebook and Twitter buttons.

16. Now that your survey is sent, your
audience will have to answer. To see
what your recipients said, click on
Responses.
