HEALTH ASSESSMENT
We, the students of BSN1-I, were tasked to perpetrate the activity entitled
“Gordon’s FHQ Interview and Narrative Report” as part of our laboratory activity and
performance task. The said activity will help us understand more about the functional
health pattern of the patient by assessing them in a form of interview. Moreover, our
interview skill and our theoretical knowledge on the topics; Gordon’s 11 Functional
Health Pattern and History Taking will serve as our stepping stone in order for us to

2. Nutrition and Metabolic The patient indicated that she doesn't have any food
Pattern or medication allergies. She eats meals three times a
day without a snack in between, and she consumes
six to eight glasses (1480–1920 mL) of water daily.
The patient prepares her own food that depends on
her cravings; as she described eating "cooked,
grilled, steamed, and boiled foods," "I'm the one who
prepares my food to monitor and maintain my diet,"
as verbalized by the patient. The doctor has not
prescribed any specific dietary restrictions for the
patient, with the exception of salty, spicy, and fatty
foods.
3. Elimination Pattern The patient usually voids urine 4-6 times a day, with
a yellow-to-transparent color depending on her water
intake. She usually defecates three times a day and
verbalizes the urge to defecate six minutes after
entering the comfort room. According to the patient,
she hasn’t experienced any voiding or defecating
problems. However, she used a laxative drug to
increase bowel movement, as prescribed by the
doctor.

6. Sleep-Rest Pattern The patient normally sleeps 5–6 hours a day, and her
earliest time to go to sleep is at 12:00 am. She wakes
up at 5:00 a.m., depending on the schoolwork that
she needs to finish. “I’m suffering from insomnia;
that’s why it is really difficult for me to sleep, plus
the pain in my surgical incision,” as verbalized by
the patient. However, she mentioned that she doesn’t
take any medication to promote sleep. According to
the patient, she finds it hard to go back to sleep if she
wakes up unexpectedly. Sometimes, the patient takes
2 hours of rest to relax and watch some movies.
7. Self Perception/ Self The patient was able to express her condition and
concept Pattern feelings. According to the patient, she feels positive
about her education status, as she loves learning, but
she feels negative about her financial status.
Furthermore, “I have been questioning myself about
whether I am on track, my values, and many more,”
as verbalized by the patient. She has anxiety and
self-consciousness, particularly when she lacks
confidence. Nonetheless, the things that made her
stressed were problems caused by pressure to study
and by her family.
8. Role relationship Pattern The patient plays the role of the youngest child in a
family of seven individuals. She mentioned that she
lived with her parents and was responsible for the

10. Coping/ Stress Tolerance The patient copes with stress by playing songs and
Pattern reading books when she has problems with her
boyfriend. Moreover, “I used to talk to my family
and friends to ask for some guidance if I had serious
problems,” as mentioned by the patient. She’s facing
financial problems that are causing her to feel
stressed. The patient stated, “I guess it slowed my
recovery process,” in response to the impact of stress
on her. She does not use any medications to treat her
emotional or mental problems.
11. Value- Belief Pattern The patient's religious affiliation is Roman Catholic,
and she and her family attend holy mass every
Sunday. In response to the influence of her belief in
self-care, the patient stated, "They taught me to
believe and have fear of God as I used to do."
Furthermore, the patient believes in quack doctors
and the usage of herbal plants for treatment.
