Annual Health Check-up

Blue Yonder is committed and focused towards the wellness of our associates. All associates
who are 35 years of age and above are encouraged to get their annual health check-up and
claim reimbursement for the same.

Frequency
• An eligible associate can undergo Health Check once during a calendar year (Jan-Dec)

Process
• Choose a diagnostic center of your choice
• Pay and claim reimbursement of a bill amount upto INR 3000 in a calendar year
• Complete the Health check reimbursement form and submit along with original bill to the
accounts team

Recommended Tests

• Complete Haemogram • Electrocardiogram (ECG)

• Fasting blood sugar • Consultation with Physician
• Post Prandial blood sugar • Consultation with Ophthalmologist
• Lipid profile • Consultation with Gynecologist
• Total Bilirubin • Consultation with Dentist
• Serum Creatinine • Ultrasound scan of Abdomen
• Serum uric acid • Chest X –Ray
• Blood grouping Rh Factor • PAP Smear for women
• Urine Routine • Any other tests prescribed by the Doctor
