Reports reached this office that:

a. State previous incident of any negligent act

b. State previous incident of any negligent act
c. State previous incident of any negligent act, and
d. On October 13, 2023, at around 4:00 p.m., customer, Irene
Bello of Block 4 Lot 30 paid through Gcash the amount of
₱______as reconnection fee, and you were advised of the same
but you did not register as basis for future billing the
application, the customer’s name, and meter into the system.

The foregoing if proven true after investigation are negligent acts

which may result to a possible disciplinary action being imposed upon
you or to your termination for failure to qualify for regularization due
to unsatisfactory performance during probationary period.

You are hereby given the opportunity to explain the foregoing in

writing within five (5) days upon receipt hereof. Should you fail to
submit an explanation, we will consider it as a waiver of your right to
explain and the management will then decide based on available
records.
