2. This is to inform you that the proceedings under Prohibition of Benami Property
Transaction Act, 1988 are on-going in your case. Hence you are requested to provide
the following details:

a. Please provide a brief note on the Nature of business activity carried out by
you from 01.04.2016 to till date.

b. Copy of the ROI along with the computation of income from A.Y 2016-17 to till
date and copy of assessment order passed if any u/s.143(3)/144
r.w.s.143(3)/148/153A/153C of Income Tax Act, 1961 from A.Y 2016-17 to till date.

c. Please furnish the copy of capital account, balance sheet and Income &
Expenditure Account/Profit & Loss A/c. from A.Y 2016-17 to till date.

d. Please furnish the bank account number, bank name, bank account branch in
your name and in the name of your family members.

e. Please provide the details of Immovable and Movable property (i.e. Any
Deposit, Insurance Policy, Holding in Quoted/Unquoted Shares, Mutual Fund
Holding, Fixed Deposits, Jewellery/Bullion and any other ornaments and Vehicles)

4. The above-mentioned information is called for u/s. 21 of the Prohibition of

Benami Property Transaction Act, 1988. Any delay in furnishing the details as
called for, the penal actions under section 54A are liable to be under the
Prohibition of Benami Property Transaction Act, 1988.
