COMPETENCY APPRAISAL EXAMINATION GUIDELINES


For Students

Pursuant to the University policy on the conduct of the Competency Appraisal


Examination, all examinees are reminded of the following:

GENERAL GUIDELINES

1. you must arrive at the testing area at least 1 hour before your scheduled
examination. This will allow you time for admission, storage of personal items, to
use the washroom, attend to your personal necessities etc. Once the examination
has started, late comers shall not be allowed to take the examination, and must
wait for the next offering of CA. Morning session shall commence at exactly
8:00AM, and 1:30PM for the afternoon session;

2. no personal items, including cellular phones, hand-held gadgets or other


electronic devices, watches, wallets, purses, hats (and other head coverings except
for Hijab), bags, jackets, books and notes are allowed in the testing room. The test
administrator is not responsible for lost, stolen or misplaced personal items. You
shall only be allowed to bring the following during the scheduled examination,
borrowing of any of these items is strictly prohibited:
 valid school identification card (without unnecessary object in it);
 transparent envelope;
 one (1) black ball-point pen (ball pen);
 maximum of two (2) number 2 pencil;
 one (1) eraser;
 one (1) sharpener;
 one (1) non-programmable calculator (only CEE, CAE & CAFAE);
 one (1) handout (only for CHE 503/BCHE 603 & ARCH 503);
 snacks (secured in a transparent zip lock bag); and
 transparent water bottle (without label).

3. you must wear the following during the examination, failure to comply, you shall
not be allowed to proceed with the examination and must wait for the next
semester’s offering of the CA:
 white polo/t-shirt/blouse (without print or label);
 jeans/skirt (without belt);
 rubber shoes/sandals (or any comfortable footwear);
 white Hijab for Muslim students; and
 prescribed OJT uniform for Nursing students.

***Note: Wearing of belt and other unnecessary accessories such as watch, etc. are
prohibited.

For Students

4. you must occupy the seats assigned to you in your respective testing room. Your
seat number and classroom assignment shall be provided to you in advance and
will also be posted outside the testing room;

5. you must present yourself to the examiner/proctor for inspection, including your
pockets and the transparent envelope (containing the items stated in #1);

6. you shall attend to your personal needs before the start of the examination, to
maximize the time allotted, you shall refrain from going out of the testing room.
Once the examination has started, you are only allowed to go to the comfort room
twice (2) and must be accompanied by the assigned staff. During the LAST hour of
the examination, you shall not be allowed to leave the testing room;

7. you shall always put your answer sheet in your desk where it is visible to the
proctor; You may not remove copies of the examination questionnaire from the
testing room and may not share or discuss the questions or answers seen in your
exam with other examinees;

8. you are responsible of keeping the confidentiality and integrity of any answer you
have taken. Actual, intended, or attempted deception, dishonesty, and/or any
action of cheating observed in an examinee shall not be tolerated. An examinee
proven to violate the policy shall be dealt accordingly;

9. you are expressly prohibited from disclosing, publishing, reproducing, or


transmitting any confidential information, in whole or in part, in any form or by
any means, oral or written, electronic or mechanical, for any purpose to any
person(s);

10. if you experience problems or distractions that affect your ability to take the exam,
notify the proctor immediately by raising your hand. If you have other questions
or concerns, raise your hand and the proctor will assist you as long as other
examinees are not disturbed. The proctor cannot answer questions related to
exam content, please reserve your question for your CA coordinator/Program
Head for clarification after the examination;

11. to ensure a high level of security throughout your examination experience, you
will be monitored at all times;

12. when the course/subject time limit ends, you must stop answering, and your test
papers must be arranged as follows.:
 answer sheet;
 test questionnaire; and

For Students

 scratch paper (ALL scratch papers shall be collected).

***Note: DO NOT insert the answer sheets in the questionnaire.

13. you must leave the testing room at once as soon as your examination has ended
and proceed to the designated waiting area;

PROHIBITED ACTIVITIES/ACTS

14. giving the room examiner or proctor and other staff members money, food, or any
other form of gratitude;

15. unusual body movements or language and signaling; loitering, talking or


discussing answers inside the testing room or along the corridor while the
examination is in progress;

16. putting any of the following markings on your examination questionnaires and
scratch papers: name, seat number, and unnecessary words or phrases; taking out
the test questionnaire used or pages thereof outside the testing room, copying,
and/or divulging the nature or content of any examination question or answer to
any individual or entity;

17. copying or referring to any solution, answer, or work of another examinee or


allowing anyone to copy or refer to your work, helping or asking help from any
person, or communicating with anyone by means of words, signs, gestures, codes
and other similar acts which enable examinees to exchange, impart or acquire
confidential information;

18. bringing books, notes, review materials and other printed materials containing
principles or excerpts thereof, coded data/information/ formula which is relevant
to or connected with the examination subject PROGRAMMABLE CALCULATORS,
CELLULAR PHONES, beeper, portable personal computers or other similar
gadgets/devices). For some specific program clusters (Arch 503), students are
allowed to bring a binder copy of Rule 7 and 8 of the National Building Code of the
Philippines where highlighting and tabbing is also permitted; For ChE 602/BChE
502 and ChE 603/BChE 503 clusters, students are allowed to use Perry’s
handbook. However, no formulas should be written in the handbook and note that
the periodic table is not allowed;

19. when the examination has already started, latecomers will NO longer be allowed
to take the exam. Examinees who are late or absent on the day of the exam will be
taking the exam in the next semester’s schedule. The abovementioned acts shall
be considered cheating and/or acts of dishonesty. Examinees whose violation is

For Students

warranted by the personnel in charge will be subjected to investigation by the


academic committee. Corresponding punishment and consequence will be
executed as stipulated in the Student Handbook; and

20. you understand this Competency Appraisal Examination Guidelines, if so, you
must conform to its terms and conditions. If observed to be violating the above
policies, appropriate action will be taken which may include invalidation of your
examination results.
