MANIFESTATION with MOTION TO ADMIT ADDITIONAL

EVIDENCE

Complainant, and unto this Honorable Office, most

respectfully state that:

1. That during the submission of pertinent documents

or evidences to prove the veracity of the complaint
submitted by the undersigned and upon checking of
the papers attached therewith it comes to our
attention that the medico-legal certificate was
absent.

2. In view of, I, the complainant herein this case,

would like to manifest before this honorable office,
to insert in my complaint the Medico-Legal
Certificate dated March 26, 2024 and Medical
Certificate dated March 25, 2024 duly signed by DR.
KRISTINE B. CHUMACERA and DR. REYMART R.
FERNANDEZ which are marked as ANNEX “A” and
“A-1”, to form part hereof, both assigned at Jose
Reyes Memorial Medical Center to support my claim
stipulated under my complaint affidavit submitted
last March 14, 2024.
 3. This motion is filed in good faith and not to delay
the hearing and solely for the foregoing reason.

PRAYER

WHEREFORE, premises considered, and in the

interest of substantial justice it is humbly prayed that the
instant Manifestation be granted in support with any
claims set forth therein.

Other reliefs be granted as shall be deemed just and

equitable in the premises.
