Congratulations! VIVAGE Senior Living is delighted to extend the following offer of employment to you!
At VIVAGE Senior Living, we strive to live up to our mission as a senior health care, solution driven

We are pleased to offer you the role of: Registered Nurse.

Your rate of pay will be: $38.50 per hour. We are paid semi-monthly (24 pay periods).

Bonus

You will receive a sign-on bonus of $5,000.00, with a 3-year commitment. If you leave before 3 years, a
pro-rated payment will be required. To receive this bonus, you will be required to sign a copy of the
Registered Nurse Agreement included with this letter. You do not need to sign this document now, but
please review for your information. We will execute this agreement once we have finalized your start date.

Benefits

You will be eligible to participate in our benefit plans, including health, dental, vision, and more. You will
also be eligible to participate in our time away from work programs based on company policy.

You will be an at-will employee with no contract or guarantee of continued employment, and either you
or the company can terminate your employment at any time and our benefit packages are subject to
change.
