On 12 April 2018, the Board of Directors and Stockholders of _________ resolved to shorten

Revenue (“BIR”) – Revenue District Office No. 50, effective immediately and request for a Tax
Clearance Certificate from your good office.

1. Two (2) copies of the Verification Slip;

2. Two (2) copies of BIR Form 1905;
3. Photocopy of the Certificate of Registration;
4. Two (2) copies of the Inventory List of Unused/Expired Principal and Supplementary
Receips and Invoices;
5. Income Tax Return – FY 2016 and 2017;
6. Certificate of Busines Retirement issued by the Office of the City Treasurer – Makati;
and
7. Board Resolution shortening the corporate term of ___________ to expire on 30
April 2018.

Atty. ______________ is hereby authorized to sign, execute and deliver any and all documents as
may be necessary for the prompt processing of this request, and to receive the Tax Clearance
Certificate and such other documents intended for __________.
