I, Maria Francesca Eleazar Hebron , owner of T&P Lucky 6 Consumer Goods

Trading with TIN 409-121-619-000 and address at G/F Savana Commercial Building Venecia
Street Corner Metropolitan Avenue, Vito Cruz, 1205, City of Makati, Fourth District
Philippines would like to request in your good office to accept and approve the Company’s
application for Deactivation of old EFPS account.

Our intention to deactivate the Company’s old EFPS account is due to non-retrieval of the
EFPS account details and information (i.e., username, password, email, and authorized
representative) from the previous accountant who registered initially in the EFPS system.

Furthermore, while waiting for the deactivation of the EFPS account, I am requesting to
accept my manual filing and payment.

We would greatly appreciate your consideration and look forward for your favorable
