Gentlemen:

I, , Filipino, of legal age, provide my consent to Global Process Manager, Inc. (“GPMI”),
further to my employment with GPMI, and in compliance with the Data Privacy Act (DPA) of
2012, and its Implementing Rules and Regulations (IRR) and other relevant policies including
issuances of the National Privacy Commission (NPC).

I give my clear consent to GPMI and its authorized

representatives-

To collect, process, store and update, or disclose my personal and/or sensitive personal
information in connection with my employment as an Position:

I agree and authorize GPMI to:

1. Contact principals and clients for Reference check or Employment Verification.

2. Continue to use my personal information in the processing of Appointment/Employment
Letter pertaining to the terms and conditions of my employment and, other relevant
documentation and processes.
3. Retain my information for a period of seven (7) years from the date of termination of my
last employment, or at such time that I submit a written cancellation of this consent with
any of the substantial justifications as provided for under the law. I agree that my
information will be deleted/destroyed after this period.
4. Retain my personal information shared with GPMI and its Principals in print or in
digital version.
5. Share my personal information and/or sensitive personal information to third parties for
any legitimate business purpose including but not limited to pre-employment
requirements, travel documentation, employment benefits administration, and government
reporting and payment contribution requirement. This includes disclosure of my
personal information and/or sensitive personal information pursuant to Data Sharing
Agreement/s between GPMI and its clients, as well as cross-border data transfers. I am
assured that security systems are employed to protect my personal information.
6. Inform me in a timely manner (within 72 hours or as approved by the NPC) if the data I
have
provided had been compromised.

I provide my consent to the GPMI and its authorized representative to process and peruse my
Personal Data (Service Personal Data) and other all relevant information, documents and
certificates required for employment-related purpose. Further, I am providing my consent out of
my own free will and without any coercion.

I also acknowledge and warrant that I have acquired the consent from all parties relevant to this
consent and hold free and harmless and indemnify GPMI from any complaint, suit, or damages,
which
any party may file or claim in relation to my consent.
