(Hereinafter the "WORKERS")

1 DEFINITIONS
1.1 “Manpower Agency” means 1ST Dynamic Personnel Resources, Inc. a duly licensed manpower agency which
recruits, select, propose, hire and deploy Filipino Workers for and in behalf of the foreign principal, employer of the
workers.

1.2 "Foreign Principal" means managers and employers; employing Filipino workers duly accredited and registered In the
Philippine Overseas and Employment Administration (POEA) under 1st Dynamic Personnel Resources Inc.,

1.3 "Personal Data" means any and all information (past, current and future) for and/or in connection with a workers
including but not limited to such individual's name(s), photos, telephone number(s), address(s}, email address(s) , family
status, passport, professional and/or employment history/track records (including but not limited to any and all CV
related data, candidate information and video and/or audio interview recordings), ethnic origin, health data, medical
records, criminal records, credit history, bank account details, tax records/identification numbers, remuneration details,
background checks (including but not limited to personal and/or professional references, criminal record checks, credit
scoring/ratings and/or debt collection register information) and any other similar information whatsoever, whether or
not available in the public domain and whether or not provided by the individual or was otherwise legally obtained.

2 NOTIFICATION
2.1 I, the undersigned Workers hereby acknowledge and confirm that I have carefully read and understood the provisions of
the Philippine Data Privacy Act of 2012 and it’s Implementing Rules including the Regulations. I further hereby consent to
and without any reservation agree that (i) the 1st Dynamic (ii) Foreign Principal (iii) any direct or indirect parent and/or
subsidiary and/or associated and/or affiliated company of the 1st Dynamic, Foreign Principal, (hereinafter the
"Company• and collectively the "Companies”) may each collect, capture, use, retain, store, structure, retrieve,
disseminate, disclose, delete, transfer, cross border transfer, share and/or process my Personal Data for the following
purposes:
(a) administering my employment, or my application therefor, including with respect to my personal information
and other related and applicable information pertaining to my employment to 1st Dynamic Personnel
Resources Inc., and to its foreign Principal;
(b) as required to comply with any laws, regulations or contracts applicable to any of the Companies or their
businesses and/or for the exercise or defense of legal claims and/o in accordance with any applicable limitation
periods and/or be held in the Companies' records pending threatened and/or commenced legal proceedings
and/or challenges;
(c) where necessary to protect my vital interests; and
2.2 I understand that my personal data will be stored in the Companies· databases, personal computer and otherwise
processed for the above purposes, in connection with, and during the period of my current period of employment
and thereafter for such further period (if any) as is necessary to fulfil the abovementioned purposes. I also
understand that the abovementioned processing activities may involve the cross border transfer of my personal
data from foreign Principal to any of the other Companies and/or any other third parties. This may involve the
transfer of my personal data to jurisdictions whose data protection laws do not provide equivalent protection to
my home country subject to the Companies and/or such other third parties taking appropriate measures to ensure
that my personal data is kept secure at all times. To the extent that my personal data includes sensitive personal
data, I hereby consent 1st Dynamic Personnel Resources Inc., processing such sensitive personal data as stated
above. I hereby agree to the cross-border transfer of my Personal Data.
