but since you have not returned, we are sending you this reminder.

Your screening test showed some abnormal changes on your cervix (entrance of the womb)
requiring you to make another visit for _____________ [further diagnosis/treatment]. (If Pap
abnormality is not invasive cancer, you may add: The changes are not indicative of cancer but,
if left untreated, they may develop into cancer in the future.)

We request that you come as soon as possible in the next two weeks so that we can give you
