agriculture and fisheries extension services of the Province of Capiz. We will use the information
given by all respondents to improve planning, public policy, governance, and government
programs and projects related to agriculture in our province and country. The interview will take
about one (1) hour to complete. All information that you provide us will remain strictly private
and confidential. We will not discuss or disclose your individual answers with anyone outside
the research team.

May we respectfully inform you that your decision to take part in this survey is voluntary – this
means that you do not have to answer these questions if you do not wish to. Whether you take
part or not will not affect any future services that you receive from the Department of
Agriculture, its attached agencies and the Capiz State University. Additionally, you may decline
to answer any question or withdraw from the interview without giving a reason. May I continue
