Subject: INCIDENT REPORT

On ____(date)___, at 0530 in the morning, our company driver Edshel Doble was backing up the Toyota
Fortuner with plate number LAI 8338 from its parking shed. On its way out, the front wheels of the
aforementioned vehicle slipped suddenly and hit the post of the parking shed. This was due to heavy
rainfall the night before the incident which makes the muddy driveway. The slip resulted to minor
scratch on the left side of the front bumper.

Subject: INCIDENT REPORT

On ____ (date) ___, at 1900 in the evening, Roger H. Pacatang was backing up the Toyota Fortuner with
plate number LAI 8338 from its parking shed in front of Admin Building of MKAVI-1. The night was so
foggy that it reduced the visibility of the driver. As he was backing up, the right side mirror of the vehicle
accidentally hit the parking shed post which resulted to minor scratches on the side mirror of the said
vehicle.
