UK INDIVIDUAL VISA REQUIREMENTS

GENERAL REQUIREMENTS

1. Duly filled-up visa application form (signed)

2. Two passport sized colored picture with white back ground(must be taken within the last six months; please do not staple the photos
on the form)
3. Original passport (must be valid at least 6 months from the end of the approved duration stay; must contain at least 2 blank visa
pages)
4. Copy of valid passport together as old passports (inside front cover containing holder’s personal information and photo; inside back
cover containing important reminders, signature and other passport information)
5. Copy of all visas (valid and expired)
6. Original personal bank certificate (must include account type, current balance, account opening date)
7. Original bank statement showing cash flow from the last 6 months, bank books, latest international credit card statements (last 6
months)
8. Copy of recent personal income tax return
9. Birth certificate
10. Marriage certificate (if travelling with spouse)
11. Print out of confirmed round trip air travel reservation (do not yet buy an airline ticket. The embassy cannot be held responsible for
any financial losses in case you have to return a ticket)
12. Detailed day-to-day itinerary (planned trip / schedule of activities)
13. Proof of accommodation (booking confirmation / reservation containing the complete name and contact information of the hotel with
hotel voucher)
14. International travel insurance (must have a coverage of at least eur30,000 / USD50,000 / Php 2.5m to cover expenses for personal
accident, urgent medical treatment, urgent admission in a hospital or urgent repatriation on medical grounds. The validity period of
the insurance coverage must be for the entire duration of the intended stay including entry and exit.

Original employment certificate with leave of absence (must include applicant’s position, date hired, salary, date of return to work)

Copy of PRC/IBP ID(if professional)
Copy of company ID

FOR BUSINESSMAN
Copy of business registration (DTI or SEC)
Copy of business permit or mayor’s permit
Copy of income tax return
Copy of financial statements

FOR STUDENTS/MINORS
Proof of enrollment
Copy of school ID
Leave of absence from school (if travelling while classes are ongoing)
Consent form (signed by both parents, allowing the child to travel)

FOR RETIREE
Copy of senior citizen card or retirement documents

FOR FOREIGN NATIONALS

Copy of alien certificate of registration (ACR)
Copy of international card residence (ICR)
Re-entry permit

Guarantee letter from company
Original bank certificate and bank statements of the company (latest 6 months)
