AUSTRALIA INDIVIDUAL VISA REQUIREMENTS

GENERAL REQUIREMENTS

1. Form 1419 – Application for a Visitor Visa – Tourist Stream completed in English Link

2. Form 956 – Advice by a migration agent/exempt person of providing immigration assistance Link
3. Copy of your passport including the photo page of your passport and any pages with amendments, endorsements, visas and entry /
exit stamps.
4. One (1) passport-sized photo (head and shoulders only against a plain background).
5. Birth certificate issued by the Philippine Statistics Authority (PSA)
6. Bank certificate showing the average daily balance for the last three (3) months
7. Bank statement/s showing account transactions for the last three (3) months.

SPECIAL REQUIREMENTS

FOR EMPLOYEES
Original employment certificate with leave of absence (must include applicant’s position, date hired, salary, date of return to work)
Copy of PRC/IBP ID (if professional)
Copy of company ID
3 months pay slip

FOR BUSINESSMEN
Copy of business registration (DTI or SEC)
Copy of business permit or mayor’s permit
Copy of business tax return for the last 2 years and deposit slip as evidence of tax payments
Copy of financial statements

FOR STUDENTS/MINORS
Proof of enrollment
Copy of school ID
Leave of absence from school (if travelling while classes are ongoing)
For children travelling with one parent:
Duly accomplished Form 1229 by non-travelling parent, and parent’s contact information and valid government-issued ID with
signature.
For children travelling with someone other than a parent:
Duly accomplished Form 1229 by both parents, and both parents’ contact information and valid government-issued ID with
signature
Duly accomplished Form 1257 by the person who will be responsible for you in Australia and valid government ID.

FOR RETIREE
Copy of senior citizen card or retirement documents

FOR FOREIGN NATIONALS

Copy of alien certificate of registration (ACR)
Copy of international card residence (ICR)
Re-entry permit

IF THE COMPANY WILL SHOULDER THE TRIP

Guarantee letter from company
Original bank certificate and bank statements of the company (last 6 months)
Copy of business documents and ITR of the company
All original documents must have a photocopy.
Note: additional documents may be required by the embassy without prior notice.
