Skill station- Measuring blood pressure. Situation Mrs rani , a 26 year- old
primi gravida , has come today for her 2nd antenatal visit at 24 weeks of
pregnancy .you have already conducted the history and have found nothing
abnormal now you will demonstrate measuring BP as part of physical
examination.

Observation – observe if the participant is performing the following steps of


measuring blood pressure in their correct sequence (as necessary) and technique.
Score”1” for each point conducted incorrectly done calculate the score .

3. Check for any crack and leakage in the bulb and

6. Place the apparatus on a horizontal surface at the
person’s heart level
7. The mercury is at the observe’s eye level
8. Ties the cuff1 inch above the elbow placing both
the tube in front
9. Raise the pressure of the cuff to 30mmhg above
the level at which pulse is no longer felt.
10. Release pressure slowly and listen with
stethoscope keeping it on brachial artery at the
elbow
11. Notes the reading where the sound heard(systolic
pressure)
12. Follows the sound and notes reading where the
sound disappear (diastolic )
13. Deflates and remove the cuff ,close the mercury
column knob

14. Inforn the women the finding and wahes hands


15. Record the reading women health card.

Situation- Mrs rajni a 28 year old , gravida 2 as confirmed her pregnanacy by
urine pregnancy test at home and has come today to your centre for her
antenatal visit and registration.
You have already conducted the history and have found everything normal. now
you will demonstrate measuring pulse as part of physical examination .

Observation- Observe if the participant is performing the following steps of


measuring pulse in their correct sequence(as necessary and techniques.
Score 1 for each point conducted correctly or marked zero if the task is not done
or incorrectly and calculate the score.

position.
2. Palpate (feels) the women radial pulse by
placing the finger tip off three middle finger
or her wrist ,below her thumb.
3. Assess the women’s pulse for regularity
and strength.
4. Look at the second hand watch or a clock;
 If pulse is regular count for 1full
minutes,
 If pulse is regular, count pulse for 30
seconds and multiply by 2.
5. Discuss findings with the women.
6. Record findings on the women’s chart.
7. After assessment of vital signs ,wash hands
throughly with soap and water, and dry
them with a clean dry cloth or air dry

CARDIAC PULMONARY RESUSCITATION
Instructions pf the examiner-
OBJECTIVES-
The station is designed to the test the candidate’s ability to-
 Assess the safety of the mannequin.
 Position of the mannequin.
 Assess the circulation, airway, breathing of the mannequin.
 Perform CPR effectively.
 Document the care giver’

OBSERVATION-
Observe if the participant is performing the following steps in the correct
sequence when performing CPR .inform the examiner that ‘vital parameters have
improved and are stable’ or provide help when he/she complete 3-5 cycles of
CPR.

SCORE-
Score the task based on the following level of perfections-
 Score’1’ for each step conducted correctly (good skills).
 Score’0’ if the task is not done or done with mistakes.
 Calculate the total score.

1. Assess the safety of the patient.
2. Postions of the patients supine on a flat surface.
3. Assess the patients’s response by asking ”you
are okay?”.
4. Assesses breathing by looking at the chest of the
movement.
5. If the patient is not breathing send or call for
help.
6. Assess circulation by palpating for the carotid
pulse .if not felt,start compression immediately.
7. Chest compressions
Identifies the centre of the chest by placing the
hand between the nipples.
8. Places the heel of the first hand over the lower
half of the sternum and 2nd over the 1st .
9. Interlocks the fingers.
10. Keeps the elbow straight and locked.
11. Compresses the chest down 5 cm quickly and
forcibly 30times at a rate of 100/minutes.
12. Observes for the return of chest movement after
each compressions.
13. Does not change position or lift hands between
the compressions .
14. Open airway and assess the airway .use head
tilt/chin lift method.-
Keep one hands over the patients
forehead ;apply form pressure and tilt the head
back.
Place over the other hand under the bony
prominence of the lower jaw an the lift the jaw
to bring it forword.

15. Mouth to mouth breathing/ambu bag;-


Keeps the airway open ,pinch the nose; take a
deep breath.
Forms at airtight seal around the patient’s
mouth and ventilates with two full breath or use
ambu bag ,delivers each breath for one second
(compression ;breath=(30:2),

16. Repeats the previous steps until help arrives or


until return of spontaneous breath-whichever is
earlier.
17. Sanitize hands .
18. Documents the care given.

INSTRUCTIONS TO THE STUDENTS

You have 5 minutes to perform CPR on the mannequin .conduct a


preliminary assessment and perform basic cardiac life support .continue
until the examiner states that ‘ vital parameters have improved and are
stable’ use the articles which are near the bedside .
Document the care given.
Hand over the response sheet to the examine.
