This is to narrate an incident that transpired during an emergency Caesarian
Section to my patient Charisa Aplaca on June 2, 2019. Prior to her stat procedure, she was mounted to delivery table for a possible normal delivery and so Dr. Javier had performed an episiotomy on her. However, it did not progress so she was immediately transferred to OR1 theater for a stat CS. After the procedure finished and after all the final counting of sponges, ATRs and instruments was done, we positioned the patient on lithotomy for episiotomy repair. Dr. Javier mentioned that she left 1 OS vaginally to be removed at 6pm. I informed the Resident on Duty to put it in his order also. On the other hand, I also endorsed the said order to the ward nurse Ms. Janice.
