INSTRUCTIONS
1. Please answer each question clearly and completely. Write in block CAPITAL letters. Read carefully and follow
all directions. If you need more space, attach additional pages of the same size.

2. The following documents must be attached with this application:-

a. Six copies of recent passport size photograph with your name / father’s name on the back and duly
attested by your Principal / Headmaster or Government Officer of Grade 17 / above or your Town Nazim. One of
these photographs to be affixed on the space provided in the Form.

b. Attested photocopy of Matriculation or equivalent certificate.

c. Attested photocopy of the Matriculation / equivalent marks sheet.

d. Attested photocopy of Domicile Certificate.

e. Attested photocopy of National Identity Cards of the candidate, his father guardian (if other than father)
and all the witnesses signing this Form.

f. Attested photocopy of father’s CDC / SSB (for sons of seafarers only).

3. Only selected candidates will be called for medical examination.

4. Any change of postal address should be promptly notified to the Officer Incharge, Seamen Training Wing.

5. Incomplete applications or applications received after due date will not be entertained.

6. Original certificates / documents listed at para 2 above are to be submitted to the Officer Incharge at the time of
joining the Seamen Training Wing and at any other stage whenever asked.

7. Application form duly completed should be returned by REGISTERED A/D POST or by COURIER SERVICE to
OFFICER INCHARGE, SEAMEN TRAINING WING, PAKISTAN MARINE ACADEMY, HAWKS BAY ROAD, KARACHI-
24, Postal Code 75780.

8. Candidate found to have submitted forged documents will be disqualified for selection. They will be liable to
dismissal without any reimbursement of fee / training charges if the forgery is established after their selection.

WARNING

All educational certificates and other documents will be verified from respective issuing authorities. In case of any forgery,
the candidate will be expelled from the course and the case may be handed over to concerned authorities for legal action.
