Dear Parent/Caregiver

As we prepare our grade 8 students for a successful transition into secondary school, our
elementary school team will engage in conversations with our secondary school partners to
understand and support each student as they prepare to attend their secondary school in the
fall.

As part of this transition process, parents, and students are invited to contribute valuable
information by participating in a conversation between elementary and secondary school teams
or alternatively, by providing written input that will be shared at these meetings. This information
will assist us to understand your child’s strengths and areas for growth as they move to a
secondary program.

Accompanying this letter, you will find the Transition Form where you can indicate:

● Whether you would like to join your child’s transition meeting either in person or virtually

● Provide written feedback to be included in the meeting

● Acknowledge that the meeting will take place and you will receive a copy of the meeting
notes.

Please return the completed transition form to your child’s grade 8 teacher as soon as possible.
