T1W, T2W Sagittal MR Myelogram T2W axial
STIR Coronal – SI joints

OBSERVATION:

Partial sacralization of L5 vertebral body is noted.


Degenerative changes are noted involving visualized vertebrae in the form of marginal
osteophytes formation and multilevel disc desiccation.
L1-L2 level shows central annular tear with posterior disc bulge indenting anterior thecal sac
without significant nerve roots compression.
L2-L3 level shows central annular tear with posterior disc bulge indenting anterior thecal sac
and causing bilateral lateral recess narrowing indenting bilateral traversing nerve roots.
L3-L4 level shows mild posterior disc bulge indenting anterior thecal sac without significant
nerve roots compression.
L4-L5 level shows central annular tear with posterior disc bulge indenting anterior thecal sac
and causing bilateral lateral recess narrowing indenting bilateral traversing nerve roots and
bilateral neural foramina narrowing indenting bilateral exiting nerve roots.
The ligamentum flavum thickness is within normal limits.
The pedicles, laminae spinous process and transverse process of the lumbar vertebrae show
normal morphology.
Lumbar canal AP diameter at L1 to L5 levels:
Level L1-L2 L2-L3 L3-L4 L4-L5 L5-S1
AP cms 1.5 1.5 1.5 1.0 1.1

The spinal cord, conus medullaris and the sub arachnoid space are normal.
The paraspinal soft tissues appear normal.
SACROILIAC JOINTS:
Bilateral SI joints show degenerative changes.
