23, Male. 24/7 mind-numbing head and face pressure. No medication has helped.

Height: 1.82

Weight: about 80 to 85 kg

Age: 23

Male

Smoker (1 pack a day ever since i was 17)

Symptoms:

1.1. Persistent, incessant head pressure and face pressure, ever since February. It started after a weight
lifting session. I suspect the headache gets worse with movement (when I walk, run, etc). This is a

1.2. Significant cognitive decline that gets worse with time. It's been getting harder and harder to read
and remember things, people around me have noticed also, and told me afterwards. My grades have
also dropped because of this.

1.3. Restlesness, inability to sit down and be calm.

1.4. Involuntary movements in the lats area, when i move my shoulders down, they start shaking
uncontrollably (both sides).
2. Previous treatments:

2.1. In April I had a consult with a Neurologist after my general doctor refered me to him, due to OTC
medication not having effect of my symptoms, he then prescribed me 25 mg of amitryptiline (it had no
effect on my symptoms) before bed and told me to get an MRI.

2.2. In June I had the MRI done https://imgur.com/a/J3rUhR9 , and I had a consult with him. I told him
that the treatment wasn't having any effects on my symptoms. He then gave me 100mg of Ketoprofen
every 8 hours for 10 days and to keep taking amytriptiline and told me there was nothing wrong with me
because the MRI was clear and the ischemic lesions found were benign. The treatment had no effect, but
when I reached out to my Neurologist, he didn't answer.

2.3. Considering the neurologist told me there was no evidence suggesting that i had a brain issue, I then
decided to go to my Intern Doctor. She told me to take the following X rays:

bloodwork and a comment i made about having a small nodule in the left side of my neck, she told me to
get a soft tissue ultrasound (https://imgur.com/a/Pm8LFlQ) She also put me on a course of antibiotics for
H pylori whichw as ultimately eradicated and on 75 mg of Pregabalin on top of the amitryptiline that I
was taking. She also told me to see a new neurologist.

2.4. I, then, saw another neurologist in July, who increased my dose of amytriptiline to 150mg before
bed and told me to get an electromyography ( https://pastebin.com/1taxdfiV ,
https://imgur.com/a/Yzb0r84 ) and to go back after the results.

2.5. I did not go with him after I got the results, because the Doctor who did the electromyography and is
a Physiatrist suggested it might be a cervical problem, and I would benefit from physiotherapy and anti
inflammatories.After she called my General Doctor, they agreed I should do 60 sessions of specialized
physiotherapy and they told me I might have myofascial syndrome. I did 10 of the 60 sessions with
absolutely no change, if anything, i was only feeling sore. I stopped because the Physiatrist told me my
neck and back seemed fine, yet the pain/pressure in my head and face was the same)

2.6. I then went to another neurologist, who put me on 50 mg of Indomethacin before bed for one week
and then to go back with him. This had no effect on my symptoms nor my pain/pressure. He then, 5 days
ago, said i should take amytriptiline every night (25mg), 550 mg of naproxen in case the pain gets really
bad and 250 mg of valproic acid every 12 hours. He also told me to get an MRA. The latest treatment he
prescribed, is having no effect on my symptoms and I'm waiting to get the MRA to get back to him.

3. Previous diagnoses.

I got an endoscopy and was recently diagnosed with barrett's esophagus in early september and i take
40 mg of esomeprazole every 12 hours daily.

Irritable bowel syndrome.

I had my gallbladder removed in 2019.

TL;DR:8 months lasting head pressure, tried a bunch of medication, none worked, need help.
