Instructions for Completing & Submitting your

New Member Questionnaire Package

Once you have downloaded this New Member Questionnaire (NMQ) Package, which
contains:

1. This Instruction Page

2. The Declaration of Intention Affirmation (DOIA)
3. The New Member Survey (NMS)
4. Notes/Explanation for the New Member Survey.
5. The Essay and New Member Questionnaire (NMQ)
6. The instructions for the PreScreen Test (PST)

...take your time to read through the entire contents before you get started. Please take
your time to write your essay and to answer the NMQ questions from your authentic self,
not as if you are in school trying to pass a test. This is mainly for your benefit; it is a
process of self-revelation, so it is best approached that way.

Also, take your time and focus on following all instructions carefully. Anyone who does
not follow the instructions will have their documents returned for correction. We, at
PTDO are looking for your being, your passion and your commitment, and we look
forward to you finding your brilliance. We are also expecting that you are self-
responsible and very capable of completing your application package with quality that
merits success! Submitting any of the NMQ related documents that are either incorrectly
completed or files that are incorrectly named, is an example of being far from brilliant
and will not be accepted, so please take the time to fully engage with what is written and
think it through and do it right the first time. Therefore, focus on following all directions
exactly.

*NOTE: If you have any technical, physical or linguistic challenges and you feel that
you may not have completed and sent in the documents as instructed, but you feel that
you are really in alignment with and keen to join the society, when you send in your
documents, please explain in that email what your situation is, and we will see what we
can do to assist you further.

A. New Member Questionnaire Package

1. Read fully our PTDO Declaration of Intention and make sure you are in
alignment with it before you proceed. If you are in alignment with our
Declaration of Intention, print out and sign the DOIA document.
2. If you are not in alignment with our Declaration of Intention, it is best that
you do not proceed with this application.

 3. Complete the New Member Survey – Read the notes and explanation
document that accompanies the New Member Survey before competing.
4. Read the instructions in the main Essay/NMQ document and complete
these parts which are the main substance of this application process.
5. Read the instructions for the PreScreen Test, and complete the test as
directed.

B. You must submit your documents in a single email. Please send from the email
that you entered into your Enrollment Application Form on the website.

C. Send Email:
o To: new.member@panterrapca.org
o Subject: Your Name-NMQ
o Message: Include your name, and a short greeting and any other short
message you wish to convey, including any issues you may have had with
completing and sending/naming the documents.

Documents to Send and How to Send Them

1. Declaration of Intention Affirmation (DOIA)

2. After you have printed and signed it, scan this document as a PDF, and name
it: John Henry Doe-DOIA (using your name)
3. New Member Survey (NMS)
4. Fill out electronically and save as a PDF; OR alternatively, you can print it and
fill it out by hand, then scan it and save as a PDF, and name:
5. John Henry Doe-NMS (using your name)
6. Essay. Part Three of the NMQ is an Essay, where you answer the question:
“Who are you?”
a. Write this in a new, separate document (full instructions are in the NMQ
document) and when finished, save as a PDF.
b. For the document header, use your name, your location and year of birth
as the document title: John Henry Doe, New York, USA, 1969
(This helps our Evaluation Team get more of a feeling of who and where
you are. Note: there is no judgment regarding your age.)
c. Name this document file: John Henry Doe-Essay (using your name)
7. Pre-Screen Test.
a. Follow the instructions in the PST carefully.
b. If you wish to do the test in another language other than English, go to this
website and download in your preferred language:

 c. Scan all the pages of your pre-screen test as a single PDF, and name it:
John Henry Doe-PST (using your name).
8. NMQ Questions/Answers.
a. Fill out the rest of the Full Participation Questionnaire per the instructions
found within the main NMQ Document.
b. Save as a PDF.
c. Name the file: John Henry Doe-NMQ (using your name)

You should now have FIVE PDF documents named thus:

1. John Henry Doe-DOIA
2. John Henry Doe-NMS
3. John Henry Doe-Essay
4. John Henry Doe-PST
5. John Henry Doe-NMQ

Email them to us as described in instruction C above.

Remember, this is in large part, an inner reflection and self-evaluation, so don’t make
it like a test, instead enjoy it, and see yourself grow.
