PERMIT FOR THE TINTED GLASSES ON THE BEHALF OF MEDICAL ISSUES

The undersigned is directed after full verification from the medical experts as within written
undertaking that the applicant is having serious issues due to sunlight and it can cause dangerous for
the health of applicant so he the details mentioned below is allowed to use the tinted glasses within the
vehicle which details are registered below with details and the following citizen is responsible for any
miss use or extra judicial activity in the vehicle the department has permitted this only for 365 days
from the issuance date the permit would be renew after the expiry of the following and would be renew
upon the behalf of medical status of the said time.
