As a prospective Sports Coach and/or Fitness Instructor, knowing your clients’ goals, medical history
and fitness levels is vital to build a better rapport and ultimately guide your client towards achieving
those goals. This may be accomplished by utilising a number of laboratory-based and field-based fitness
tests. However, before any fitness testing is carried out, it is important to conduct a detailed Health
Screening Procedure to ensure that it is safe for your client to participate in any physical activity. Your
local fitness centre has requested your assistance in carrying out Health Screening tests with their
clients.

Part A
You are to find a client and assess him/her with the use of a health screening questionnaire and different
health monitoring tests. As part of the preparation phase, you are required to:

1. Create a Health Screening Questionnaire ensuring that the questionnaire includes all the relevant
sections discussed during lectures (Personal Information, Lifestyle and Physical Activity
Information, Health Screening Questions, Client’s, Trainer’s and Witness Signatures). (Part of
R&U4 – 2.1 - 3 marks)

2. Aid your client in filling in the Health Screening Questionnaire. (Part of R&U4 – 2.1 - 2 marks)

Part B
Once you have finalised the above documents you are required to carry out a 15-minute appointment
with your client during lecture hours wherein you will:

1. Check your client's heart rate and blood pressure and calculate his waist-to-hip ratio and Body
Mass Index (BMI). (A&A1 – 2.2 - 7 marks)

2. Results obtained from the above-mentioned health monitoring tests are to be adequately recorded
on a previously prepared ‘Health Monitoring Results Sheet’. (R&U5 – 2.3 - 5 marks)

3. All the results obtained from the above-mentioned health monitoring tests need to be
interpreted by using normative data charts. The interpretation of results needs to be clearly
indicated in the ‘Health Monitoring Results Sheet’. (A&A2 – 2.4 - 7 marks)

It is important that, for assignment purposes, you compile a file with all the information gathered. As listed
below, you are requested to submit:

• A filled in copy of the Health Screening Questionnaire

• The ‘Health Monitoring Results Sheet’
• All normative data charts used

For future reference, keep a copy of the filled in Health Screening questionnaire.
The above meeting will be observed by the lecturer, who will be assessing you via a standard criteria
checklist.
