Notes:
The patient mentioned above arrived at Al Emadi Hospital emergency
room with signs of ischaemic stroke. MRI was conducted to determine
the nature of the stroke during the first hour upon arrival.
Post determination of a minor cerebral infarction in right brain necrotic
tissue, the patient was injected with 90 mg of Activase Alteplase
administration for the purpose of stroke dissolving with success.
Patient was advised to remain under clinical observation for 48 hours
for monitoring purposes.
For medical review purposes, the patient was advised to revisit the
neurology clinic for examination on 5/14/2022.
Patient revisited the neurology clinic for follow up on 5/14/2022.
A comprehensive neurologic examination has been done with no signs of
either nerve nor brain damage.
Patient advised to follow prescribed medication until the end of June to
ensure avoidance of a second stroke.
Number of physiotherapy sessions reduced to 2 sessions based on
physiotherapist instructions, and shall be scheduled upon next 2 weeks.
The patient shall resume ordinary daily activities including office duties by
the end of this week, and must avoid tension and stress or any other
physical exertion.
