I Manvi Rajesh Nichani son/daughter of Soumya Rajesh Nichani do, hereby state, declare and
undertake a solemn affirmation as follows:

1. I say that I have been admitted in School of Physiotherapy at the Dy Patil Deemed to be
University Nerul, Navi Mumbai in the academic year 2023-24.
2. I have been informed by the Centralised Admissions Office regarding the rules and
regulations of seeking admission in the University.
3. I have been clearly told and I understand the rule of Dy Patil Deemed to be University that
the University DOES NOT TAKE DONATIONS/CAPITATION in any form for securing admission
in the University.
4. I hereby agree that I have not paid any donation/capitation in any form to the Centralised
Admissions Office of the Dy Patil Deemed to be University in order to take admission in the
course of Bachelor of Physiotherapy being offered by the university.
5. I solemnly affirm and declare each and every thing stated above true and binding.
