0158 Benigno Aquino Avenue, Tangos, Baliwag, Bulacan, after having been
duly sworn in accordance with law, hereby depose and state:

1. That I am the accused in this case;

2. That I have caused the preparation of the foregoing Motion for

Reconsideration;

3. I have read the contents of the Motion for Reconsideration and

attest that (a) all the allegations contained therein are true and
correct based on my personal knowledge, or based on authentic
records, (b) the said pleading is not filed to harass, cause
necessary delay, or needlessly increase the cost of litigation; and
(c) the factual allegations therein have evidentiary support, or if so
specifically identified, will likewise have evidentiary support after a
reasonable opportunity for discovery;

4. I hereby certify that (a) I have not heretofore commenced any

actions or filed any claim involving the same issues in any court,
tribunal or quasi-judicial agency and to the best of my knowledge,
no such other action or claim is pending therein and (b) if I should
learn thereafter learn that the same or similar action or claim has
been filed or is pending, I shall report that fact within five (5)
calendar days therefrom to this Honorable Court;
